Patient Right(s) and Responsibilities 

Patients have the right: 

  1. To be treated with respect, consideration, and dignity

  2. To know the names and credentials of the health care professionals serving them

  3. To privacy

  4. To confidential treatment of all communication and records relating to their care.  Except as required and/or allowed by law, their written permission is required before we may give information to anyone not connected with their care.  

  5. To receive accurate and understandable information about their health

  6. To know the beneficial effects, side effects, and problems of all forms of treatment.

  7. To participate in decisions involving their health care and choosing a form of treatment.

  8. To be informed of services available at 国产AV Student Health Center and any related costs

  9. To know what provisions are available for after hours and emergency care

  10. To consent to, or refuse, any care or treatment

  11. To select and/or change their health care provider

  12. To review their medical records with a health care provider

  13. To receive appropriate referrals to other health care professionals and services

  14. To designate someone to make health care decisions for them.  This requires a legal document known as an advance directive.  This would be used if the patient becomes unable to make medical care decisions or prefers someone else to make decisions for them

Patients have the RESPONSIBILITY:

  1. To seek medical attention promptly

  2. To provide complete and accurate information to the best of their ability about their medical health, any medications, including over-the-counter products and dietary supplements, and any allergies or sensitivities

  3. To ask about anything they do not understand

  4. To report any significant changes in symptoms or failure to improve

  5. To respect clinic policies

  6. To be respectful of all health care professionals and staff, as well as other patients

  7. To keep appointments or cancel in a timely manner

  8. To seek non-emergency care during regular hours

  9. To know the names, purposes, and effects of medications prescribed to them

  10. To pay promptly any bills that they have incurred

  11. To follow the treatment plan and to advise the health care provider if they do not think that they will be able to follow it. It is the patient’s responsibility to be aware of the consequences of not following the recommended treatment plan.
